The agency provides professional, compassionate services designed to help people access care, build stability, develop independence, and improve their quality of life.

Outpatient Therapy provides support for individuals, groups, and families who need help with mental health, emotional, behavioral, social, or substance-use concerns. Services may include clinical assessments, diagnostic assessments, individual therapy, group therapy, family therapy, substance abuse counseling, medication management, cognitive behavioral therapy, person-centered planning, motivational interviewing, crisis response, case management, coordination of care, community resources, independent living skills, and prescreening assessments.

Peer Support Services assist individuals with communication, education, service identification, referral, and ongoing support. These services help consumers move through change and work toward self-sufficiency. Peer Support may help individuals access medical, social, educational, and other needed services while building confidence, stability, and independence.

Community Support Teams help Medicaid-eligible individuals gain access to doctors, social services, educational resources, jobs, and other needed services. Support may include assessment, planning, problem solving, identifying resources, case management, referral, linkage, monitoring, and follow-up. The goal is to help individuals remain stable, connected, and supported in the community.

Intensive In-Home Services are team-based services for children and adolescents with serious emotional, behavioral, or substance-use needs. These services are designed to help youth remain stable in the community, support the family, and prevent more restrictive placements. Services may include assessment, case management, referral, linkage, monitoring, follow-up, crisis support, and structured therapeutic interventions.

The Substance Abuse Intensive Outpatient Program, also known as SAIOP, is a structured program for adults who need support achieving and maintaining recovery. Services may include individual counseling and support, group counseling and support, biochemical assays, family counseling, training, life skills, disease management, crisis contingency, developmental disabilities support, relapse prevention, peer support services, treatment support activities, case management, community resources, food referrals, job and resume building, transportation assistance, and light refreshments. Outpatient therapy is also available. Referrals may come from counselors, therapists, physicians, hospitals, community agencies, families, or individual consumers.

Case Management includes the development and periodic revision of a care plan based on assessment information. The care plan identifies goals, needs, and action steps related to medical, social, educational, behavioral, emotional, and other service needs. Case Management helps individuals stay connected to the right resources and support systems.

Referral and Linkage includes helping individuals connect with the services and providers they need. This may include scheduling appointments, coordinating with medical providers, linking individuals to social services, connecting with educational programs, and helping people access community-based supports that address identified needs.

Monitoring and Follow-Up includes ongoing contact to make sure the care plan is being implemented and that the individual’s needs are being addressed. Follow-up may involve the individual, family members, service providers, community agencies, or other support systems.
